MINIMUM QUALIFICATIONS:
 
CCI BWO: Master’s degree in social work, psychology, education counseling or related field OR Bachelors degree with two years of childcare experience.  Director who is qualified by education may perform the duties of the Human Service Professional.  Minimum of 1 HSP for every 16 children.  One HSP for every 30 children unless assigned referral and intake duties, then maximum of 16 children.

Must be privileged in Crisis Prevention Intervention.  Must have competency in RCCL, DJJ, DHS, CARF, Amerigroup, RBWO and Vashti Center Policy and Procedures.  Must be privileged to administer medication.  Must be certified in First Aid/CPR.  Must have ability to effectively communicate with consumers and families using written and verbal skills.  HSP should be able to develop consumer’s oversight plans and ensure that the plan is progressing and goals and objectives are being documented.
 
 
GENERAL DESCRIPTION:
 
The HSP is responsible for providing oversight of services to Vashti consumers and their families.  The HSP is responsible for monitoring the consumer’s needs and ensuring that appropriate services are being provided and arranged to meet those needs.  Duties include, but are not limited to: the coordination of the facility’s admission evaluation; implementation of permanency goals, the development of the service and Room, Board, Watchful Oversight plans; case work services as provided in the consumer’s service plans; and monitoring of the consumer’s education and/or vocational needs.
 
SERVICE RESPONSIBILITIES:
The facility’s admission evaluation shall be coordinated by the consumer’s designated HSP along with the Program Director seeking external consultation as appropriate to assure consumer meets admission criteria.  The HSP shall assess the needs of the consumer in the areas of health care, room, board and watchful oversight, education, family relationships, personal, social and vocational development, and any behavioral issues that require monitoring.

If accepted into the RBWO program, the HSP will be present during the admission with the Intake Coordinator and acquire the necessary information needed to develop the service and room, board and watchful oversight plan to include medication issues and the completion of the medication admission sheet assuring medication count with signature of placing authority.

A service and room, board and watchful oversight plan shall be developed by the consumer’s HSP within 30 days of admission and will be reviewed with the milieu staff with particular attention to safety concerns. The plan shall contain the following data:

The results of the assessment and identified needs statement of time limited goals and objectives for the consumer and family and methods of achieving and evaluating and will also include MRO services and service provider.
Statements of activities to be followed by the consumer and staff members in pursuit of the stated goals and objectives.  Assure any special occurrence either positive or negative effecting consumer’s Vashti program is documented in consumer’s progress notes.
Statements of any special care and services that will be arranged for or provided directly.  Initiate school placement with CSW and oversee education progress.  Develop alternative education activity for those expelled or awaiting school placement enrollment.
Statement should address goals and preliminary plans for discharge and permanency plan to include custody order in DFCS/DJJ case plan and commitment order.  Short and long term RBWO goals shall be addressed as well.
Statements about the types of discipline that should be employed.  This will be done during the orientation of the consumer with the HSP and will include consumer rights, resident handbook, management interventions, ESI interventions with the nurse, chain of command, grievance and permanency by the legal placing agency.  Any medical restrictions on the use of any disciplinary ESI to include CRR shall be documented and shared with all care staff.

The service and room, board and watchful oversight plan shall be updated by the HSP at a minimum of every six months and pertinent progress notes and data shall be incorporated in the plan to measure attainment of stated goals and objectives.
The HSP along with Program Directors and Behavior Managers shall oversee the implementing of the service and room, board and watchful oversight plan.
The HSP will complete Monthly Summary Reports by the 10th of each month for the previous month and distribute accordingly.
The HSP will complete the “ECEM” Every Child Every Month report by the 15th of each month assuring completion within 48 hours of meeting with the consumer and distribute accordingly.
The HSP will perform monthly reviews of each consumer’s record and insure formalized permanency plan, short and long term goals are consumer’s chart and documenting all permanency goals and their progressive status.
During a consumer’s placement, the preliminary plans for discharge shall be adjusted according to the consumer’s circumstances.  The HSP will alert the MRO of projected discharges at least 30 days in advance and initiate discharge preparation meeting to formalize discharge organization.  At least 3 months prior to planned discharge, except in cases of emergency discharges, the HSP shall formulate an aftercare plan that identifies the supports, room, board, and watchful oversight summary and resources that the consumer and their family are expected to need following discharge.  When a consumer is being discharged for placement in another institution or similar program, the receiving institution or program, except in cases of emergency discharges, shall be given at least 30 days notice of the proposed date of placement. At discharge, the HSP will assure placing agency or assigned designee completes all discharge material to include:

Sign off on receipt of clothing, personal items and money
Complete medication discharge sheet
Provide remaining medication as prescribed except for OTC medications
Provide prescription for next 30 days of medication
Assure Vashti has discharge address and contact information
Direct consumer’s surrogate to MRO to finalize MRO discharge/transfer for mental health service.

In emergency, provides at least 72 hours prior notice of discharge to the parents, guardians or placement agency.  If such notice is not possible, it shall be so documented.
Provides a complete written discharge summary within 30 days of discharge to include:

The name, address, telephone number and relationship of the person or entity to whom the consumer was discharged, or the name of the placing agency if discharged to a placement agency
A summary of all the services and room, board and watchful oversight activities provided for the child to meet assessed needs while the consumer was in the institution
A summary of the consumer’s and the family’s goals and objectives and accomplishments during care
A summary of any problems encountered by the consumer and family during care
A summary of assessed needs which were not met during care, and a summary of the reasons why they were not met
A copy of the completed discharge summary shall be made available to the consumer’s parents, guardian or placement agency representative when it is completed.
All consumers shall receive casework services as provided in their service plan from their assigned HSP who shall meet with and counsel the consumer.  The results shall be recorded in the consumer’s case records.  The purposes of such services are to identify and monitor the consumer and families’ progress relative to the needs, goals and objectives being encountered by or with the children in care.
The HSP shall monitor the consumer’s educational or vocational progress in the course of providing casework services and planning.  Progress reports, such as report cards, and other records or documentation of a child’s educational or vocational performance while residing in the institution shall be maintained in the child’s case records.
Children attending public schools who wish to participate in extracurricular activities shall be provided such reasonable opportunities in accordance to the consumer’s service plan, guardians consent and supervision restrictions.

In collaboration with the RBWO records technician, monitors the content of reports, records, and reviews.
The HSP will attend meetings as deemed necessary by the CEO and Program Director.  Attendance at 2nd and 4th weekly Multi-Team meetings is required so information can be shared between the MRO and RBWO.  Such information shall be documented in the RBWO record.
